Helping Hand Nurse, LLC is seeking a Registered Nurse (RN) to serve as a Per Diem Field Supervisor for our home health patients. Helping Hand Nurse is a home health care agency. We provide nonmedical and medical services.

Responsibilities:

  • Field Supervision:
    • Perform home visits to monitor the quality of care provided by home care staff.
    • Provide ongoing guidance, support, and training to field staff, including RNs, LPNs, and caregivers.
    • Conduct supervisory visits as required to ensure compliance with care plans and regulations.
  • Client Assessments:
    • Perform initial and periodic in-home assessments of clients to determine care needs.
    • Develop, review, and update individualized care plans in collaboration with the care team.
    • Communicate changes in client conditions to the care team and adjust plans accordingly.
  • Compliance and Quality Assurance:
    • Ensure adherence to all state and federal regulations, company policies, and professional standards of practice.
    • Document all activities, including client interactions, assessments, and field visits, accurately and in a timely manner.
    • Participate in quality improvement initiatives and assist in resolving client or staff concerns.
  • Coordination and Communication:
    • Act as a liaison between clients, families, and the care team to ensure effective communication.
    • Collaborate with the office team to schedule visits, manage documentation, and provide feedback.
    • Promote a culture of safety, excellence, and accountability.
    • Qualifications:

      • PA RN License OR Multi-State License
      • Computer savvy
      • Minimum of 2 years of nursing experience, preferably in home health, hospice, or community health settings.
      • Valid driver’s license and reliable transportation.
